Treatment Agreement

This page describes how therapy at SarvCare works in practice: who you're working with, what to expect from sessions, how payment and cancellations are handled, what's confidential, and what happens if something doesn't work out. It's the agreement we both rely on when we work together. You'll be asked to confirm it as part of the intake form before our first paid session.

1. About your psychologist
SarvCare is a solo online practice run by Atena Karimzadeh, basispsycholoog. Atena holds a WO Master's in Clinical Psychology from Universiteit Leiden and a Bachelor's in Psychology from the University of Tehran. She is registered with the Nederlands Instituut van Psychologen as Psycholoog NIP® (#250722) and is bound by the NIP Beroepscode and tuchtrecht.
Atena is not registered in the BIG register, and does not use protected titles such as gz-psycholoog or psychotherapeut. The clinical work is the same evidence-based therapy you would expect from any qualified psychologist.

2. Nature of services
SarvCare offers online individual therapy in English and Farsi to adults (18+) experiencing mild to moderate mental health concerns. Sessions are conducted via Google Meet over an encrypted video connection. The work draws on evidence-based methods, primarily Cognitive Behavioural Therapy (CBT) and Schema Therapy, adapted to your individual situation.
3. What's not in scope
SarvCare is not the right fit for the following situations. If your needs fall outside our scope, Atena will, where possible, help you find appropriate care.
- Emergency or crisis care
- Active suicidal thoughts or recent suicide attempts
- Active psychosis or mania
- Severe eating disorders requiring specialised treatment
- Anyone needing inpatient care
- Court-mandated or forensic therapy
- Children and adolescents (under 18)
- Medication management (we don't prescribe; we refer to a psychiatrist when indicated)

4. Sessions and fees
Sessions are 50 minutes, online via Google Meet. The first 30-minute consultation is free and without obligation. After that, we work with a sliding scale so therapy stays accessible. You're trusted to choose the rate that honestly reflects your situation; no documentation is required.
- Standard rate · €85–95 — for people in stable professional circumstances
- Reduced rate · €60–75 — available on request, no explanation needed
- Accessibility rate · €40–50 — for students, recent arrivals adjusting to life in Europe, and people in genuine financial hardship; a limited number of slots are available at this rate
Once agreed, the rate stays fixed unless your circumstances change and we decide to revisit it together.
5. Payment
After each session you'll receive an invoice payable via iDEAL. Payment is due within 7 days of the invoice date, or before our next session, whichever comes first. The free 30-minute consultation is not invoiced.
SarvCare invoices as exempt under Article 11.1.g Wet OB (medische vrijstelling), so no BTW is charged. The invoice will include the practice's KvK number and Atena's NIP number, which is what supplementary or international insurers typically need if you intend to claim reimbursement.
6. Cancellation policy
- The free 30-minute initial consultation can be rescheduled or cancelled at any time without charge.
- Regular sessions must be cancelled at least 24 hours in advance.
- Cancellations within the 24-hour window, or no-shows, are charged in full, except in genuine emergencies.
7. Confidentiality
Everything you share in sessions is confidential. As a NIP-registered psychologist, Atena is bound by professional secrecy under the NIP Beroepscode. Information will not be shared with third parties without your explicit consent, with the following legal exceptions:
- Immediate danger to your safety or the safety of others.
- Suspected child abuse or domestic violence, where Atena is required to follow the Meldcode huiselijk geweld en kindermishandeling.
- A court order requiring disclosure.
8. Continuity and coverage
SarvCare is a solo practice. For planned absences (vacation, training), Atena will let you know in advance. For unplanned absences:
- Contact your GP (huisarts) for non-urgent matters, or your local huisartsenpost outside office hours.
- Call 112 in case of emergency.
- Call 113 Zelfmoordpreventie (0800-0113) for a suicidal crisis.
Where appropriate, Atena can refer you to colleague Psycholoog NIP practitioners for continuity of care during longer absences.
9. Records
Atena keeps a confidential dossier per client, as required by the WGBO. Records are stored encrypted and handled in line with NEN 7510-compatible practices. They are retained for 20 years after the last session, as required by Dutch healthcare law. Financial records (invoices) are retained for 7 years under Dutch tax law.

10. Complaints
If something doesn't feel right, please raise it with us first — most concerns can be resolved through a conversation. If we can't resolve it together, you have access to an independent klachtenfunctionaris and, if needed, the Geschillencommissie Vrijgevestigde GGZ. Full details on the Complaints Procedure page.
11. Termination
Either of us can end the therapy relationship at any time. Where possible, a closing session is preferred so we can wrap up the work and, if useful, talk through next steps or a referral. If you stop responding for an extended period, Atena will reach out once to check in, and may then close the file.
